Picture it: Incomplete records are stacked everywhere, some in permanent files, some on the floor. Most are unaccounted for and have gone uncounted for years. That's the scenario Rose T. Dunn, RHIA, CPA, FACHE, FHFMA, chief operation officer of First Class Solutions in St. Louis, and her team happened upon at one facility.
