'Frequent fliers' in appeals process called a serious problem by OIG

In a recent report from the HHS OIG on appeals of Medicare claims, many Administrative Law Judge (ALJ) staff raised concerns about what the report calls "frequent filers"-providers who file far more than the average number of appeals.
