In a bid to tamp down hospital admissions associated with congestive heart failure, some insurers and employers are monitoring patient vitals remotely and seeing favorable results. But will these programs scale to other chronic conditions?

The ROI is compelling, but why couldn't this be achieved with a simple, 2-way text message sent to the patient? Removing the expense of equipment would improve the ROI.
