A clarification on the Hospital Safety Score: Leapfrog data is used only when it is available. When not available, it uses public CMS data only. The score issues an A,B,C,D,or F for hospital's propensity for errors, injuries, accidents, and other safety problems. And we list all hospitals and all measures for those hospitals, not just the top performers.Look for updated scores to be released in November, 2012.
