Illinois has fined health insurance company Quartz $500,000 for failing to cover substance use disorder and mental health treatments at adequate levels. Quartz sells health insurance plans on the Affordable Care Act exchange at healthcare.gov, and off the exchange, in parts of northwestern Illinois.
