First, congratulations! I think you are the perfect choice for this very difficult job -- you have the experience, the imagination, and you are still (and plan to continue as) a bedside doctor, although that may be difficult for a bit. The objective, as I understand it, is to reduce the cost of healthcare (destroy the "tapeworm") without compromising quality.
