NHS warned to cut errors in hospitals and surgeries

According to the annual report by the Healthcare Commission, 3,471 patients were known to have died over the past year as a result of medical errors. Now, NHS in England is struggling to attain an acceptable standard of safety in hospitals and GP surgeries. Huge improvements have been made, but NHS boards do not yet systematically investigate why one in 10 patients admitted to hospital suffers harm as a result of an error. Officials hope to create a national database of never events.
